5D Mark II Importing Files Err 02

Hello Everyone,

Recently i'm having this weird issue. My 5d mk II and 64gb sandisk extreme pro card is having err 02 while importing video files that more than 1 min into my mac via usb with camera.

I've tried to format the card in camera, it doesnt work. the err 02 remain.
So I tried format the card in my mac first and then format in camera again, the err 02 still remain.

Finally,I tried to format the camera in my mac with exFAT format.
but since 5d mk II doesnt support exFAT format, so after i insert the card back into the camera.
the camera lcd appears " please format card in camera. "
So i formatted the card once again in camera and finally it works.

But soon after i used the card to record raw files, the err 02 issue appears again.
Same thing, it wont let me import my normal h.264 video files that more than 1 minute.
